POSITION TITLE: School-Based Counselor
REPORTS TO: Behavioral Health Director/Assistant Director
POSITION SUMMARY: This position is responsible for serving as an integral part of a school-based clinical team with core responsibilities consisting of providing counseling services (individual and telehealth), performing intake screenings and assessments, and creating service plans with school-aged children and adults. The person in this position will assist patients in overcoming barriers in daily life, encourage making positive life changes, and provide tools for a healthier lifestyle by utilizing best practice therapeutic techniques in their service delivery.
MAJOR DUTIES AND RESPONSIBILITIES:
- Perform screenings and assessments of new patients to determine medical necessity for treatment and complete case management assessments upon intake.
- Develop individualized treatment/service plans based on assessments, client histories, and best practice interventions
- Regularly review and modify plans as needed in collaboration with patients and interdisciplinary staff.
- Provide individual, group, and telehealth counseling services to support patients in overcoming dependencies, life adjustments, and mental health challenges.
- Deliver case management and supportive services aligned with patient needs and service plans; advocate for clients in crisis situations, including referrals for crisis stabilization, detox services, and other community resources.
- Maintain accurate and timely documentation of services, treatment progress, and outcomes in accordance with agency policies and procedures.
- Coordinate care and collaborate closely with other professionals, including behavioral health providers, nurses, medical staff, courts, probation officers, and community partners.
- Facilitate discharge planning and develop aftercare and follow-up services for patients transitioning out of treatment.
- Participate in regular interdisciplinary team meetings and meet with the supervisor for case review, documentation oversight, and professional development.
- Attend trainings to enhance clinical skills, maintain knowledge of current treatment practices, and contribute to agency standards.
- Engage with clients’ family members, as appropriate, to provide support, education, and progress updates.
- Perform additional responsibilities and duties as assigned by management.
ESSENTIAL KNOWLEDGE AND ABILITIES:
Knowledge:
Working knowledge of the nature of related treatment modalities, best practice interventions, and techniques; of different types of assessments and their uses inpatient treatment.
Abilities:
Must demonstrate the ability to utilize appropriate assessment tools to interview and assess clients, observe, record, and report on an individual’s functioning; identify community resources and services for patients and coordinate the provision of services; establish effective working relationships with internal staff and relevant community organizations; interact positively with patients and their families, work as a team member, communicate effectively, verbally and in writing, to maintain confidentiality, and work independently under general supervision.
Qualifications:
- Must possess a relevant education (Master’s degree in social work, counseling, or psychology)
- Licensure preferred (or working towards licensure).
- One-year mental health work experience preferred.
