Position: Licensed Practical Nurse
Status: Full-Time
Position Summary: Provides direct and indirect patient care in the ambulatory care setting. Provides care that meets the psychosocial, physical, and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a Registered Nurse. Maintains regulatory requirements, nursing and ambulatory care policies, procedures, and standards. Communicates with physicians and team members about changes in patient’s clinical condition, including results of diagnostic studies and symptomatology.

MAJOR DUTIES AND RESPONSIBILITIES:

• Provides direct patient care under the supervision of a RN, evaluates outcomes, consults
with other healthcare team members as required and adjusts nursing care processes as
indicated to ensure optimal patient care.
• Ability to perform a head-to-toe assessment, under the supervision of a RN, on all patients
and reassessments as per policy. This includes neonate, pediatric, adolescent, and
geriatric patients and the general patient population. This includes postoperative
assessment.
• Performs patient care responsibilities considering needs specific to the standard of care
for patient’s age.
• Ability to adequately assess and reassess pain. Utilizes appropriate pain management
techniques. Educates the patient and family regarding pain management.
• Performs all aspects of patient care in an environment that optimizes patient safety and
reduces the likelihood of medical/health care errors.
• Supports and maintains a culture of safety and quality.
• Communicates appropriately and clearly to team leader, co-workers, and physicians.
• Demonstrates the ability to assist physicians with procedures and perform services
requiring technical and manual skills under the direction of a RN.
• Treats patients and their families with respect and dignity. Identifies and addresses
psychosocial, cultural, ethnic, and religious/spiritual needs of patients and family.
• Demonstrates the ability to perform treatments and provide services to level of licensure.
• Maintains current knowledge of medications and their correct administration based on age
of the patient and his/her clinical condition.
• Ensures an adequate stock of supplies and proper functioning of equipment.
• Maintains a good working relationship within the department and with other departments.
• Manages and operates equipment safely and correctly.
• Demonstrates the ability to be flexible, organized, and function under stressful situations.
• Performs other duties as assigned by management.

Professional Requirements:
• Completes annual educational requirement.
• Maintains regulatory requirements.
• Reports to work on time and as scheduled; completes work within designated time.
• Wears identification while on duty; uses computerized punch time system correctly.
• Maintains patient confidentiality at all times.
Professional Requirements:
• Completes in services and returns in a timely fashion.
• Attends annual review and department in services, as scheduled.
• Represents the organization in a positive and professional manner.
• Actively participates in performance improvement and continuous quality improvement
• (CQI) activities.
• Complies with all organizational policies regarding ethical business practices.
• Communicates the mission, ethics, and goals of the facility, as well as the focus statement
• of the department.
Regulatory Requirements:
• Current LPN/LVN licensure in the State of West Virginia.
• Current BLS certification.
• Successful completion of an IV therapy and phlebotomy course.
• Successful completion of arrhythmia course.
