Position Title: Quality Management; Infection Preventionist; Case Management Registered Nurse          

Status: Full-Time

Position Summary: 

Quality Management Director: Responsible for planning and implementing the performance improvement program to meet the needs of the hospital.  Provides education to medical staff, hospital staff and Governing Body.  Facilitates performance improvement activities and CQI activities throughout the hospital. 

Infection Preventionist: Responsibilities include the identification, investigation, reporting, prevention and control of infections and communicable diseases within the organization, including both healthcare associated infections and community-acquired infections.

Case Management RN: Responsible for the coordination and implementation of case management strategies pursuant to the Case Management Plan. 

MAJOR DUTIES AND RESPONSIBILITIES:

  • Maintains oversight responsibility for all performance improvement activities conducted throughout the institution.
  • Develops and coordinates processes to monitor quality of care.  Identifies opportunities to improve patient care, treatment and services.
  • Demonstrates effectiveness in planning and implementing the performance improvement program to meet the needs of the hospital.
  • Responsible for facilitating performance improvement and CQI activities throughout the hospital.
  • Demonstrates effective organizational skills through ongoing interaction with clinical chairpersons, nurse managers, ancillary department managers, administrative team and Governing Body to assist in facilitating the hospitalwide PI program.
  • Compiles reports for committees and administrative team.
  • Actively participates in QM meetings, provides oversight for meeting preparation, gives presentations on performance improvement to committee members.  Presents risk management statistics and information.  Sits as member of UM Committee.
  • Prepares reports/surveys/statistics to be reported to the Governing Body, government agencies.
  • Assists in utilizing information obtained via performance improvement activities to seek and act on opportunities to improve patient care processes.
  • Designs and implements new methods to improve the efficacy and outcomes of the Infection Prevention and Control Program.
  • Demonstrates ability to teach principles and practical application of infection prevention and control to all levels of healthcare staff.
  • Demonstrates understanding of Standard Precautions and Transmission-Based Precautions as set forth by the CDC.
  • Develops and maintains a system for identifying, reporting, investigating and controlling infections and communicable diseases of patients and staff.
  • Develops, in collaboration with the medical director, clinical care guidelines for both inpatient and outpatient care settings, in addition to, process and system guidelines
  • Performs intake assessment on patient within 24 hours of admission (patients meeting screening criteria).
  • Reviews care and treatment for appropriateness against screening criteria and for infection prevention and control, quality and risk assessment; documenting same in computerized database.
  • Responsible for authorization of appropriate services for continued stay and through discharge.
  • Knowledgeable of CDC guidelines and definitions of healthcare associated infections (HAIs), accrediting organization standards, federal and state regulations and national guidelines.
  • Plans and coordinates care of the patient from pre-hospitalization through discharge.   Responsible for authorization of appropriate services for continued stay and through discharge.  Participates in performance improvement activities.
  • Perform other duties as assigned by management. 

Professional Requirements:

  • Completes annual education requirements.
  • Maintains patient confidentiality at all times.
  • Adheres to dress code, appearance is neat and clean.
  • Maintains regulatory requirements, including all federal, state, local regulations and accrediting organization standards related to the Medical Surgical Unit and, as appropriate, to the hospital.
  • Maintains and ensures patient confidentiality at all times.
  • Wears identification while on duty.
  • Represents the organization in a positive and professional manner.
  • Ensures compliance with policies and procedures regarding department operations, fire safety, emergency management and infection prevention and control.
  • Effectively and consistently communicates administrative directive to staff and encourages interactive departmental meetings and discussions.
  • Complies with all organizational policies regarding ethical business practices.
  • Communicates the mission, ethics and goals of the facility, as well as the focus statement of the department.

REGULATORY REQUIREMENTS:

  • Graduate of an accredited school of nursing. Bachelors of Nursing Degree or Degree in Health Services Administration preferred.
  • Current West Virginia Registered Nurse Licensure, or current multi-state licensure within state of practice.
  • Current BLS certification.
  • Specialized training in Infection Prevention and Control within last five (5) years; evidence of pertinent continuing education minimally every two (2) years preferred.
  • Five (5) or more years of experience in Quality Management preferred.
  • Three (3) or more years of previous clinical experience in an acute care setting required.
